When to Refer a Patient for Comprehensive Home Health

If a Patient has…

  • Newly Diagnosed Medical Conditions or Complex, Debilitating Symptoms, requiring skilled monitoring, and/or health teaching.

  • Pressure Ulcers, Open wounds, or burns

  • Ostomies

  • Newly Diagnosed or Poorly Controlled Diabetes

  • Bedbound patient requiring the teaching of family to manage dependent patient

  • Need for exercise to improve ambulation, mobility, independence, safety, and circulation

  • Medications requiring medicine teaching, monitoring, and management

  • Needs dressing changes, monitoring of wounds following surgery

  • Has hydration and nutrition problems

  • Comprehensive home care services are available on call 24 hours a day, and 7 days a week. Our on-call nurses are promptly available to assist our patients when there is a need.

  • Servicing locations in the Los Angeles, Ventura, San Bernardino, Riverside and Orange Counties.

To qualify for the services the following needs to be met—

  •  The patient is homebound and has difficulty or needs assistance to leave home.

  •  The service is prescribed by the patient's physician and is medically necessary.

  • A nursing or therapy service is part of the treatment plan.

  • A patient can receive these services through her/his insurance.
